Autumn Grace Memory Care
Catered Living for Persons with Memory Loss
Autumn Grace Memory Care Services provide specialized care for persons with memory loss and dementia related conditions. We offer spacious and comfortable living with gracious amenities.
Our Mission
Our mission is to provide our residents a positive experience in a safe and friendly environment with personal dignity a main consideration. At Autumn Grace we provide the joy of healthy daily living along with the supplemental care necessary for adults with memory-related challenges. We focus on the individual strengths and needs of our residents in order to promote the independence necessary for them to have productive and meaningful experiences. A dedication to physical, emotional, psychological, and spiritual health is the basis of our program. We strive to satisfy each individual's specific needs, with attention to family concerns and the resident's continuing care.

Licensed Facility
Autumn Grace is licensed by the State of Minnesota, Department of Health as a Housing with Services Establishment as well as a Board and Lodge facility. Autumn Grace carries a Class F & A Home Care License through the Minnesota Department of Health.
Safety and Security
Our facility provides a coded security entry and exit system for your loved ones' safety and security.
Our residents routinely engage in creative crafts.
Activity Center and Social Interaction
Autumn Grace Activity Center consists of a fully secured indoor activity center which has a beauty parlor, a multimedia room, and a fireside room with library. Church services are scheduled for various denominations.
Individualized programs offer the highest quality life enrichment experience for our residents. Each program takes into consideration each resident's functional ability. We feel that comfort, security, and the freedom to move in an unrestricted environment promotes well-being. Access to the activity center further enhances our activities program. Knowing each resident's personal story enables Autumn Grace to provide care which brings comfort and security to each individual.

- On-site RN and LPN services weekdays
- 24 x 7 on-call Registered Nursing Services
- Varying nutritional programs designed to meet individual dietary needs. Options may include (but are not limited to): pureed foods, finger foods, soft foods, no concentrated sweets, no added salt, diebetic diets
- Meal time adaptation
- Assistance with eating
- Meal set up as needed
- Assessment of needs and abilities
- Individualized treatment plans
- Safe, secure environment (security coded entry and exit)
- Referrals to other services
- Health screening and monitoring
- Companionship and support
- Involvement in social activities
- Discussion of daily current events

- Field trips to community social events

- Nutritious snacks and noon meals per doctor order and resident likes and dislikes
- Support services for residents and family members
- Bathing, grooming, toileting, and dressing
- Incontinence management (bowel and bladder care programs)
- Ambulatory care and transfers
- Monthly vitals and case management by nursing staff
- Medication assistance / management
- Laundry services
- Housekeeping services
